Cook Library Patron Survey

Cook Library wants to know what you need and how they can help. Please complete this quick, anonymous survey to let your voice count.

This short, five-minute survey will be available between March 23 and March 31. The survey is voluntary, and your answers cannot be linked to you. You may skip any questions which make you uncomfortable. You may be asked to participate more than once since this survey is available on paper and electronically for customers. If you have already completed the survey once, please do not answer the questions again.

Xeryus Johnson and Denise Brown-Rouse are the only people who will see all of the individual responses. The library will be given a report of the overall findings at the end of the survey period.
